The facts of this case is that Margaret Burns, who is 63 years old, has suffered from a occipital hemorrhagic infarct because of a stroke. Also, she has a history of rheumatoid arthritis, hypertension, osteoporosis, gastroesophageal reflux, and depression. Because of this, she has been staying at the hospital and is later transferred to a skilled nursing facility for recovery before being able to return home to live an independent life. When Margaret was at the skilled nursing facility, she develops pain and swelling on her right leg and a fever. An ultrasound is performed on her, which shows a deep vein thrombosis (DVT). She is then prepared to be transferred back to the acute care hospital to get treatment. However, in the process of doing so, the copies of her records are mixed up with a different patient. As a result of this, she is given the wrong medications, which puts her in the ICU.
One of the appropriate quality improvement tools to analyze this problem is the cause and effect diagram, which “helps to identify and organize the possible cause for a problem in a structured format” (Buchbinder & Shanks, 2017, p. 160). This diagram is good in helping to analyze any given situation, such as this case.
The first management issue in this case is the lack of attentiveness when checking the records for accuracy. They should have a nurse who is able to double check for accuracy of the records before the patient is being transferred. The second management issue is poor organizational skills on Carol Stevens part. Even though Carol had a rough morning having to deal with a flat tire on her way to work, the pile of works she has to do after arriving, and her son calling to say that he missed the bus, she lacks the ability to be able to manage her workload, time, and resources effectively. The last management issue is that the acute care hospital did not check that the records are accurate before admission. Because they did not check or ensure that everything was correct, Margaret took the wrong medications for three days, which puts her in ICU because of another stroke.
The legal and ethical obligations a healthcare organization has to its patients and families are to protect the patients that they are caring for at all cost and to do the right thing for the patients. These obligations apply to this case by Margaret suffering another stroke because the health professionals did not properly protect her by double checking to make sure that her records are correct. When the responsibility of protecting the patients are ignored, the patients will suffer as a result, and the health professionals would have to be responsible for what happens. This is why it is best to do the right thing to avoid this kind of situation.

The facts of this case are Mrs. Burns has an extensive delicate medical history and is currently presenting with a new condition that requires immediate attention. The unit secretary made a medical error and the admitting facility as well regarding not adequately checking the information sent out and receiving. As a result, Mrs. Burns received the wrong medication, which worsens her hemorrhagic stroke and is currently in ICU.

These legal and ethical obligations apply to this case as the acute facility has overlooked all of these, and neglectfully has put the patient’s medical outcome at risk. Mrs. Burns’s medical condition worsened due to a neglectful staff that did not have adequate help to ensure paperwork was correct, and the accepting facility also was negligent, not to ensure the patient information received pertain to the right patient. As a result, both facilities failed to execute the transferring and admitting process adequately in a way that ensured the patient gets the best care possible. That is why as our future role,s we must remain active a monitor all areas of our department because, in this field, an error can lead to a medical emergency.
