This assignment requires you to undertake a community assessment within the City of Horizon in order to identify potential health issues for the community. In this assignment you are also considering the role of the registered nurse in primary health care.

You will present the assignment in a report format using the template provided as a guide.

The assessment is divided into two parts – Part A & Part B. You are to address both parts.

Part A: Community Assessment ??In Part A your task is to complete an assessment of the City of Horizon using the headings, biophysical assessment, psychological assessment, physical environment, socio-cultural features, behavioural features and health system. Please consider aspects of the points listed under each of these heading in your report

1. Biophysical Assessment that outlines the demographic profile of the population. Include:

• Age profile
• Gender profile
• Ethnicity and cultural profile

2. Psychological Assessment identifying how the community is connected and its ability to respond to events. For example:

• Communication networks for example telecommunications and media (newspaper, radio, t.v. etc)
• Volunteer agencies
• Law enforcement
• Emergency services
• Social support services

3. Physical Environmentthat identifies features of the environment such as:

• Geography
• Population size
• Classification: remote, rural, urban, etc
• Climate
• Air quality
• Water source, availability and quality
• Sewage and other waste disposal
• Housing types and adequacy

4. Socio-Cultural features: This section relates to the way a community is structured and organised through a range of public institutions, resources and activities. Your assessment should consider:

• Marital status
• Family composition
• Education levels and facilities (formal and informal)
• Income
• Employment rates and occupation types
• Transport
• Leadership of community
• Religion
• Community groups and organisations

5. Behavioural features: This section relates to the interaction between community resources and the community. It includes patterns of consumption of community resources, goods and services. In this section consider:

• Access to goods and services
• Shopping facilities
• Sport and Recreation facilities
• Leisure activities
• Entertainment facilities
• Dining and eating facilities

6. Health System: Consider the availability and access to health services (inclusive of preventative and illness services) for people.

Part B: Identification of a potential health issue and the role of the registered nurse

?Following your assessment identify and discuss one (1) potential health related issue for the Horizon community and consider the role a registered nurse may have in addressing the health issue you identified.

The assignment is to be presented as a report format using the template provided. This is located in the assessment folder. Download and type your answers in – it is already formatted. Do not change the formatting.

My suggested breakdown for each part as per the template:

• Introduction:10% of final word count (200 words)
• Part A: 60% of Final word Count (1200 words): That is 200 words per heading
• Part B: 20% of Final word count (400 words)
• Conclusion: 10% of Final word count (200 words)

Introduction: An introduction should introduce the assignment. It should clearly state the purpose, aims, and objectives of the assignment, indicate any key terms and should present a plan for the discussion.

Part A: Community Assessment:

In Part A your task is to complete an assessment of the City of Horizon using the headings of biophysical assessment, psychological assessment, physical environment, socio-cultural features, behavioural features and health system. Your course outline indicates the aspects you may consider. Topic 3 also contains readings that provide more specific information.

How do I find information?

You will need to explore the City of Horizon. Do this by:

• examining the map of the City of Horizon,
• read each client’s background stories (information about Horizon is embedded in these),
• look at the Horizon City Council webiste for information, and
• also do some detective work based on what you know about Horizon and where it is situated.

Note: that you will not find “everything about everything” – this emulates “real word”. You will need to base part B on what you have found out.

Part B: Identification of a potential health issue and the role of the registered nurse

From your assessment, you are asked to identify and discuss one (1) potential health related issue for the Horizon. You will need to justify why the issue you selected is of concern for the City of Horizon. To do this you will need to refer to the academic literature.

Once you have explained what the health issue is and why it is of concern, you then need to consider the role a registered nurse may have in addressing the health issue you identified for the community. Here you are required to refer to academic literature.

Conclusion: A conclusion should summarise the main themes presented in the report and should evaluate the presented material. It needs to outline any recommendations. It should not introduce any new material. It needs to demonstrate what you have learned from the academic paper and state the conclusions drawn
