Emma is a 27-year old female who has lately been struggling with significant feelings of
worthlessness and shame due to her inability to perform as well as she always had in
the past. For the past few weeks, she has felt unusually fatigued and found increasingly
difficult to concentrate at work. Her coworkers have noticed that she is often irritable and
withdrawn, which is quite different from her typical upbeat and friendly disposition. She
has called in sick on several occasions, which is completely unlike her. On those days
she stays in bed all day, watching TV or sleeping. Her husband suggested that she see
a therapist who later recommended a combination of medications, support groups, and
exercise.
Emma is married and lives with her husband and their dog. She does not smoke
cigarettes or drink alcohol. She has a very demanding, high stress job as a second year
medical resident in a large hospital. She has always been a high achiever. She
graduated with top honors in both college and medical school. She is hoping that her
therapists’ recommendation to exercise, along with her other treatments, will help
decrease her depression and improve her mood and self-concept.
Emma is constantly on the move at work which leaves her eating meals at odd times if at
all. She is exhausted when she comes home and never feels like doing much of
anything aside from watching TV or sleeping. When they first got married, she and her
husband would participate in bowling leagues and other couples sports with friends but
with her tedious residency schedule they stopped. She would like to get into yoga
because her therapist suggested that it may improve her mood and outlook on life but
she is worried she doesn’t’t have the time to commit. Thinking about finding time to
exercise just adds more stress to her life and makes her feel overwhelmed and
depressed. Currently, Emma does not participate in a purposeful exercise program.
When asked the question, “On a scale from 1 to 10 (1 being the worst and 10 being the
best), how would you rate your overall weekly physical activity?” Emma answered with a
“2.” When asked, “Why did you not rate yourself a “1”, she replied, “I am constantly on
the go at work and log quite a few steps on my Fitbit.” Then, when asked, “What would
you need to do to answer the question with a “3?” she replied, “If I were to start going to
yoga classes at least 2 times per week, I would consider myself as a “3.”
Emma’s physical activity goal is to find an exercise regimen that is both convenient and
something she enjoys doing.
